Hypertension is a chronic disease with a high prevalence that can cause complications such as stroke and heart disease. Management of hypertension relies heavily on self-management, but many patients face obstacles such as lack of knowledge, low motivation, and minimal family support. These obstacles can interfere with blood pressure control and increase the risk of complications. The research aims to analyze predictors of barriers to self-management in hypertensive clients in the community—descriptive correlation research with a cross-sectional approach involving 60 respondents through a purposive sampling technique. Inclusion criteria are clients diagnosed with hypertension by a doctor, age > 18 years, and cooperative. Exclusion criteria were not present in the study and there were disease complications. The independent variables are motivation, family support, and knowledge. The dependent variable is hypertension self-management behavior. The research was conducted in the Juwana Community Health Center Working Area, Pati Regency in January 2025. The instruments used were HSMBQ, HBP-SCP, family support questionnaire, and HK-LS. Data analysis using tests Spearman’s Rho. Motivation has a strong relationship (r=0.628, p=0.000), while family support (r=0.550, p=0.000) and knowledge (r=0.464, p=0.000) have a moderate relationship. Motivation, family support, and knowledge have a significant relationship with hypertension self-management. Community health centers can develop interventions to improve self-management behavior in hypertensive clients through increasing motivation, family support, and knowledge.
